We start with a collaborative planning session to understand your goals, data systems, and program structure. Including your health and data teams, we ensure alignment on your key priorities to have the right foundation to build your customized Health Outcomes Analysis.
You will receive access to an interactive dashboard, updated monthly, allowing you to monitor health outcomes and track progress. You can explore trends across key attributes such as site, class, and demographics to identify strengths and challenges across subgroups. This allows you to focus efforts where they will have the greatest long-term impact.
In addition to the dashboard, you'll receive three tailored reports in a clear, presentation-ready format—delivered at 60 days, 90 days, and year-end (timing is flexible). Each report includes visual summaries and actionable insights to support health managers and program leaders.
